WHO CAN REFER?

YOU DON'T NEED A PHYSICIAN REFERRAL TO GET AN APPOINTMENT!

Parents, physicians, child care centres, junior kindergarten teachers and other early years professionals can make the referral. Parental consent is required for referrals.

WHEN CAN I REFER?

No child is too young to be referred. The deadline for referrals is December 25 of the calendar year in which a child is eligible for junior kindergarten. If your child's 4th birthday is this year, don't hesitate!

HOW DO I REFER?

♦ You can always phone our Intake Co-ordinator at (905) 688-1890, ext. 151 to make a referral or

♦ Download our Referral Package, complete the forms, then send them to our Intake Co-ordinator by mail or by fax. Use the menu on the top left to link to specific instructions.

♦ We appologize for the inconvenience but online referrals are not available to protect your privacy.
