WHO CAN REFER?

YOU DON'T NEED A PHYSICIAN REFERRAL TO GET AN APPOINTMENT!

Parents and guardians, physicians, child care centres, junior kindergarten teachers and other early years professionals can make the referral. Parental consent is required for all referrals.

WHEN CAN I REFER?

No child is too young to be referred. The deadline for referrals is December 25 of the calendar year in which a child is eligible for junior kindergarten. If your child's 4th birthday is this year, don't hesitate!

HOW DO I REFER?

♦ You can always phone our Intake Co-ordinator at (905) 688-1890, ext. 151 (for Niagara Falls, Port Colborne and Welland please use ext. 168) to make a referral or

♦ Download and mail or fax our Referral Package. Complete the forms, then send them to our Intake Co-ordinator by mail or by fax. Click this link.
